OEHS is available to assist site administrators in determining
the cause of workplace accidents and developing an action plan to
reduce the frequency of worker injuries. Site administrators may
request a Loss Analysis Report specific to a school, location, or
Division, which can help to identify problem areas. A Loss Analysis
Report is distributed to each Division on a quarterly basis, and
includes information on the number and cost of workplace injuries
by location. Accident prevention training can be arranged by

All accidents and injuries must be
investigated and reported within 24 hours of the incident. The
District’s Injury / Accident Investigation Report has been
incorporated into the Incident System Tracking Accountability Report (ISTAR). For further information on ISTAR refer to

Serious employee injuries that result in
death, disfigurement, dismemberment or hospitalization for more than
24 hours for treatment other than observation must be reported to
the California Occupational Health and Safety Administration
(Cal/OSHA) and OEHS within 8 hours. OEHS may conduct an in depth
analysis of these accidents in addition to the investigation
conducted by the site using the
OEHS Serious Accident
Procedure.
