Reporting a Serious Employee Injury to OSHA:

Cal/OSHA §342(a) will automatically assess a $5,000 fine for not reporting a “serious injury” to the nearest Cal/OSHA District Office within 8 hours of occurrence. OSHA fines are payable by the location or school that was cited. A “serious injury” is defined as an employee death, amputation, permanent disfigurement, hospitalization for more than 24 hours for other than observation, or an incident resulting in multiple injuries requiring hospitalization.

The Injury/Accident Investigation Report; Section 9: Required Referrals lists the telephone number for the Los Angeles area OSHA office. Section 3.4 of your Injury & Illness Prevention Program (IIPP) lists the closest OSHA office location for other areas; their telephone number, and information that will be required when reporting an injury.
